Applying for an insolvency procedure

Gather together all relevant documentation before you begin. For your guidance we have provided a checklist of information that you may need when completing your application.

   Complete this form to assist the Official Assignee in gaining an understanding of your affairs if you haven't already completed this form as part of your application.

   Complete this form to assist the Official Assignee in gaining an understanding of the affairs of a company in liquidation.  Every company director is required to complete and submit a company Statement of Affairs.
6. Application for an Official Assignee decision to be reviewed [98 kB PDF]
   Use this form if you disagree with the Official Assignee’s decision to reject your application and you wish you apply to have it reviewed

    Use this form if the Official Assignee has asked that you complete and submit it so that your ability to pay contributions towards your creditors can be assessed.
