   Use this form if you have decided to make yourself bankrupt.
2. Debtors application for no asset procedure [47 kB PDF]
   Use this form if you want to apply for a No Asset Procedure.
3. Debtors application for summary instalment order [54 kB PDF]
   Use this form if you wish to apply for a summary instalment order.
4. Statement of affairs [510 kB PDF]
   Complete this form to assist the Official Assignee in gaining an understanding of your affairs. Every bankrupt is required to complete and submit a Statement of Affairs.
5. Liquidation statement of affairs [100 kB PDF]
   Complete this form to assist the Official Assignee is gaining an understanding of the affairs of a company in liquidation. Every company director is required to complete and submit a company Statement of Affairs.
6. Bankruptcy application to travel overseas [13 kB PDF]
   Use this form if you wish to apply for permission to travel outside of New Zealand while bankrupt.
7. Bankruptcy sample affidavit self employment [21 kB PDF]
   Use this form to assist you in completing an application to be granted consent by the Official Assignee to be self-employed whilst bankrupt.
8. Budget form [35 kB PDF]
   Use this form if you are bankrupt and the Official Assignee has asked that you complete and submit it so that your ability to pay contributions towards your creditors can be assessed.
9. Proof of Debt form [27 kB PDF]
   Use this form if you are owed money and wish to lodge a claim.
