Choose 'Apply Now' option from website (Online Services homepage)

Alternatively you can print forms from the website or request them from 0508 467 658

Complete Statement of Affairs and choose Summary Instalment Order option

Select a Supervisor to act

Choose whether to print verification page or request it be posted to you

Complete the verification page and return it to the Insolvency and Trustee Service

Note :

This cannot be actioned until the Supervisor’s consent form has been received

Await decision by the Official Assignee
