Choose 'Apply Now' option from website (Online Services homepage)

Alternatively you can print out blank forms from the website or request them from 0508 467 658

Complete the Statement of Affairs and select the No Asset Procedure option

Choose whether to print verification page or request it be posted to you

Complete the verification page and return it to the Insolvency and Trustee Service

Await confirmation of decision by Official Assignee
